Form to be filled out by all volunteers:
Steps for Volunteer Application Form (this may vary with computer systems. Basically you want to fill it out and get it to AOR with attention to Mike McGinnis).
1) Click the link immediatley above and fill in form (SSN is optional but preferred - AOR will not disclose it except to do the background check).
2) Print out (you may want to print an extra copy to keep - on some systems completed forms cannot be saved digitally).
3) Sign form.
4) Make sure you include a photocopy of your picture ID and either:
a) Deliver to AOR Office, or
b) Mail to: AOR 110153 South Station Anchorage , AK 99511 c/o Mike McGinnis, or
d) Hand deliver to Mike.
Forms to be filled out by all parents:
Copy of player's birth certificate (submit to manager)

You will be able to post photos to the site once the teams are built in April. Please only post photos of your children and provide a copy of this photo release form for us to keep in our records. You can email a copy of the form to aor_secretary@gci.net, fax a form to the office at 868-5076, drop in the mail to us or drop it by the office.
